• Immunization Requirements

If you plan to take 6 or more credit hours you must complete the following requirements

Complete the Health and Immunization Record

Tuberculosis Test - Must be dated within 12 months of college entry

Two Measles-Mumps-Rubella (MMR) Vaccinations - You are exempt from this if born before 1/1/57

Health Insurance proof - Photocopy of the front and back of your card, attached to the Health and Immunization Record

Meningitis Information - NYS law requires that we inform you of this disease and the vaccination against it. Page six of the Health & Immunization Record has this informaiton

Tetanus booster - must be dated within the last 10 years

If you are part-time with less than 6 credits No vaccination proof or TB test is required, but please complete our Health and Immunization Record
